Role Responsibilities
The RN Care Coordinator (RNCC) is a key member of the Hospital at Home program, responsible for facilitating safe, effective, and patient-centered care transitions from inpatient settings to home-based care. Key duties include:
- Building strong professional relationships and collaborating effectively with healthcare providers and interdisciplinary team members to identify suitable patients for the program.
- Educating patients and their caregivers about the Hospital at Home model, ensuring a smooth transition and understanding of their care plan.
- Providing nursing, administrative, and operational guidance to program staff.
- Assisting with operational and business functions, managing projects, and acting as a liaison to other departments.
- Serving as a clinical expert, coordinating, and evaluating the delivery of evidence-based care aligned with national and statewide clinical guidelines.
- Facilitating access to care, leading performance improvement initiatives, and supporting development of clinical pathways.
- Ensuring cost-effective, safe, and high-quality patient care while enhancing the overall patient experience.
- Maintaining adherence to program standards and quality metrics established by the Service Line.
Qualifications
- Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- May require specialty certification.
- 2+ years of inpatient acute nursing care experience preferred.
- 1+ years of Case Management experience preferred.
Qualifications
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license
- Requires that the RN has graduated from a nationally accredited nursing program
- Requires Basic Life Support certification through the AHA
